Isolated lesion in the splenium of corpus callosum on magnetic resonance imaging (MRI) is not a rare finding. However, clinical manifestations of splenium lesion are nonspecific and variable. A 45-year old female was admitted to our hospital with a new-onset headache. The brain MRI showed a focal lesion in the splenium of the corpus callosum. Headache improved completely after symptomatic treatment. On MRI taken 3 months later, the lesion was completely resolved without specific treatment.
